You’ll be cared for by a Board-Certified Nurse Practitioner and Certified Functional Medicine Practitioner (CFMP) with 10 years of internal medicine experience, specializing in getting to the root cause of chronic issues and optimizing whole-body wellness.

• Self-pay primary care fee is due prior to scheduling your appointment and the amount insurance pays provider will be reimbursed to you once received

• It is your responsibility to check with your insurance provider to confirm your plan’s coverage, our clinic's network status with your insurance, covered and non-covered benefits before your visit. 

• We will not be responsible if your insurance is inactive or if you have not updated our clinic provider as your primary care provider (PCP) with your insurance company. 

• We will send you a holistic health screening tool prior to scheduling your appointment to make sure we are the perfect fit for you. We will send you information for other providers if we are not a good fit for your health needs. We will also need a list of all your prescription medications prior to scheduling your appointment. We do not prescribe chronic opioids, benzodiazepines, or phentermine. We do not stock any vaccines in the clinic.

We’ll also discuss important factors like your stress levels, family life, spirituality, sleep, mood, and trauma history to get a full picture of your health.
 

• We do not store vaccines in the clinic. For flu or pneumonia vaccines, please visit your local pharmacy or health department.

• Please be on time for your appointments.

• To avoid no-show or late cancellation fees, cancellations require at least 24 hours’ notice.
   
• Cancellations must be made by phone call or text — emails or patient portal messages are not accepted for cancelations.

• We do not provide medical care or consultations through patient portal messages or texts. Your concerns need to be addressed in-person or via telehealth appointments.
   
• Each visit may only cover 1-2 health issues to ensure thorough care. We will schedule you additional appointments if needed.
